Use the Case Plan screen to create or modify an offender case plan, and view a history of case plan changes and previous versions.
To create a new case plan
To access this screen from the Main Menu, select Community Case Management Case Plan.
If necessary, query in the Header block to locate the offender.
If there are previous case plan versions for the selected offender, they automatically display in the Case Plan block, in order of Create Date, with the active or most recent plan first. To see the last edit date/time/user ID stamp, hold the mouse cursor in the farthest righthand column.
Click a row in the Case Plan block, click Insert, then complete the following fields:
Number: automatically fills in after you save this file. The number will be 1 higher than the number of the previous (most recent) version of the plan for this offender. If this is the first version, the number is 1.
Status: defaults to Active; mandatory.
Location: select from the List of Values; mandatory. It displays only those locations that:
a) have a current established interest in the contact, and
b) are update-accessible in the current caseload.
Officer: select from the List of Values. It displays only officers at the selected location; mandatory.
Create Date: automatically fills in with the current date; cannot be changed.
End Date: leave blank. The end date for this case plan automatically fills in when you create a new case plan, therefore ending this case plan.
Supervision Level: automatically fills in with Pending.
Next Review Date: automatically defaults to a set number of days after the current date. The number of days is set by your TAG administrator, and is based on the offender's current supervision level. You can change the next review date using a MM\DD\YYYY format. The new date cannot be:
before the current system date; or
after the review period set by your TAG administrator.
Click Next Block to access the Assessment Summary block. In this block, you record the kind of offender assessment that was done, and information regarding the assessment.
Changes and additions to this block after saving will result in a new case plan.
Fill in:
Category: select an assessment category from the List of Values; mandatory.
Summary Text: type in a comment regarding the assessment summary; mandatory if you have selected a Category.
Click Next Block to access the Plan Detail block, where you record the case plan goals.
Changes and additions to this block after saving will result in a new case plan.
Fill in:
Goals - Type: select a type of goal from the List of Values; optional.
Goals - Sub-Type: select a goal sub-type from the List of Values; mandatory if a Type is selected.
Target Date: type in a completion date for the stated goal or objective using a MM\DD\YYYY format. .
Narrative: type any additional details; optional.
To define the steps required to achieve the goal in the selected line, click Casework Steps.
Click Save.
Deleting a plan is not permitted.
