Offender Assessment

On the Assessment screen, you fill out a questionnaire to assess an offender for management in the community, or in an institution. Each response has a numeric score, which TAG tallies to suggest a result. Depending on the type of assessment, the assessment scores can suggest a variety of outcomes like a particular offender supervision level, or specific goals for an offender.

The Assessment screen has two pages. On page 1, you select the type of assessment you will conduct, as well as view all previous offender assessments. On page 2 you fill out the questionnaire for the assessment type you selected on page 1.

  1. For institutional work, select Institutions Case Management Classification from the Main Menu.

    For community work, select Community Case Management Assessment to access this screen from the Main Menu.

  2. If necessary, query in the Header block to locate the offender.

    If there are previous assessments for the offender, they automatically display in the Internal Assessments block, listed most recent first. Last edit date/time/user ID stamp is visible through the furthest-right column.

    If there are many previous assessments, use the scrollbar to view them. When you select a previous assessment by highlighting it, the Offender's Assessments block automatically displays the associated offender assessment information.

  3. Click Next Block to access the New Assessment block. If necessary, scroll to a blank record with the down arrow key, or click Insert Record.

    In the New Assessment block, you select the type of assessment you are recording as well as the assessment date, and the re-assessment date. You can update the assessment until the re-assessment date or until the assessment is approved, whichever is sooner.

  4. Fill in the New Assessment block.

  1. Click the questionnaire button to display the questionnaire for the selected assessment type. Assessment screen page 2 will appear.

Each questionnaire may consist of a number of sections each containing questions. The sections are listed in the Select Section block at the top of screen page 2. When a section is selected, the associated questions display below. Select each section in turn, and answer the questions for each section.

Questionnaire answers may be mutually exclusive, inclusive, or automatic, depending on how each question has been set up by your TAG system administrator.

  1. Select a section in the Select Section block.

  2. Answer the questions within that section. If the list of questions is long, put your cursor in the Question field, and press the down arrow on your keyboard to scroll through the list.

  1. Answer: select an answer from the List of Values. Record the most serious selection.

  2. Score: automatically fills in when you select your answer.

  3. Comment: type in a comment about the answer; optional. This field may automatically display a comment if your TAG administrator has given the question an override.

Note: If your TAG administrator has set up your assessment to have autoscoring, then each listed question will be automatically answered by a TAG database search. The Comment field can also automatically display a comment.

  1. Repeat steps 6 and 7 until you have completed the questionnaire.

Section Score Pop-Up Window: a pop-up window may display while you are recording questionnaire answers for an offender supervision level assessment. This occurs if you reach a score in a section that is high enough to determine an offender supervision level. Click the link regarding "section scores" at the end of these instructions to learn about section scores, and to learn about the Section Details button.

  1. Click Save.

  2. You are returned to the first page of the Assessment screen. The Assessment Score, Calculated (Supervision) Level, and User ID fields are automatically filled in. The Section Details button may or may not be enabled depending on whether you achieved a section score high enough to determine an offender supervision level (see sub-topic below regarding section scores and the Section Details button).

If you need to override TAG's suggested result, go to step 11. Otherwise, skip to step 12.

  1. You can manually override TAG's suggested result.

    In particular, an override to minimum custody for select situations may of been set up by your TAG administrator. This override is set up so if a certain score is achieved for a particular question (e.g. concerning arson or sex crimes), a higher offender supervision level must be assigned. If you try to save your assessment with minimum supervision when there is this override, you will see a warning pop-up window, and you will not be able to save. You will only be able to save your assessment after you have overridden the minimum supervision level with a higher supervision level.

  1. Click Save.
