Use the Breach/Amendment Application screen to record:
an offender's breach of the conditions on an order
an application by an offender for an amendment to an order and/or the conditions on an order
a disposition (decision) regarding the offender's application for an amendment to an order and/or the conditions on an order
To access this screen from the Main Menu, select Community Orders/Requests Breach/Amendment Appl.
Query in the Header block to locate the offender.
Click Next Block to access the Breach and Amendment Application block. The Breach and Amendment Application block lists breaches and offender amendment applications by date, with the most recent first.
Fill in these fields to record a violation or amendment application:
Date: defaults to current date. To change the date, enter another using a MM\DD\YYYY format. The date must be the same as, or earlier than the current date.
Type: select the type (violation or amendment application) from the List of Values; mandatory.
Court (Location/Type/State): select the court description from the List of Values; optional. The next two fields fill automatically with the court's type and state or province.
Comment: type in a comment; optional.
Status: displays Pending until a disposition is recorded.
Click Save.
Click Next Block to access the Related Orders and Conditions block.
Fill in the Related Orders and Conditions block:
Order Line: select from the List of Values the order that the offender has breached or has applied to be amended; mandatory. The List of Values displays all existing offender orders by Line #. (If there are no orders or conditions, the list will not open.)
When you make a selection, the other fields fill with the information relating to the selected order.
Related Conditions: click on the List of Values to open the Related Conditions pop-up window. You can view conditions entered against the selected order by Status Date (most recent first), then by Type and Condition. You can also add a condition:
Click on the List of Values in the Comm Condition Code field. The Effective Date, Expiry Date, Status, and Status Date fields fill automatically when you select a condition. You can type details in the Details field. Multiple selections are permitted; optional.
Click Save.
Click Previous Block to return to the Breach/Amendment Application screen.
The information to be entered in the Disposition block will not be available at the time of the application. You will have to come back and fill in the fields once a disposition is made.
Click Save.
