(Community Intake Personal Contacts)
Use this pop-up window to add or locate existing records of persons associated with a selected offender, as personal or professional contacts or as victims or witnesses in a specific case.
Open this pop-up window by clicking the Add/Update Offender Contact button on the Personal and Professional Contacts screen, or the Person Detail button on the Victims/Witnesses screen.
When accessed from the Victims/Witnesses screen, the name selected on that screen carries over to the second block.
When accessed from the Personal and Professional Contacts screen, you must type at least the last name in the Search for Persons field, and then click Query.
If you find the person record, skip to step 6.
If no record for the person appears, select the Create New Person Record (This Name) checkbox. This copies the name to a new line in the second block.
Fill in:
Birthdate: type in the date of birth using a MM\DD\YYYY format. Optional field.
ID: TAG automatically fills in this field when you save this file.
Click Next Block to access the Person Details block., then complete the following fields:
Primary Language: select from the List of Values. Optional field.
Marital Status: select from the List of Values. Optional field.
Citizenship: select from the List of Values. Optional field.
Interpreter Required checkbox: if the primary language is one that requires an interpreter to communicate with the officer or the court, select this box. Optional field.
Criminal History: enter a short note on the person's criminal record. Optional field.
Deceased Date: if the person is deceased, enter the date of death using a MM\DD\YYYY format. Optional field.
Coroner #: enter the phone number of the coroner responsible for the deceased party. Optional field.
Gender: select from the List of Values. Optional field.
Click Save.
Click Identifiers to open the Identification pop-up window, where you can record details of the identification document (s) presented by the person.
Click Contact Numbers to open the Contact Numbers pop-up window, where you can record telephone numbers for the person.
Click the Next Block to access the Addresses History block.
If the person has the same address as the offender, select the Lives/Lived with Offender checkbox.
Click the Copy Offender's Address button. If the offender's address is a residence (Address Type RES recorded on the Addresses screen), and if the person lives with the offender, the address recorded for the offender appears in the top line.
To enter other addresses for the person, be they past or present, complete the following fields:
Unit #: type in the apartment number and/or street number of the person's current residence. Optional field.
Street: type in the name of the street where the person lives. Optional field.
City: select from the List of Values. Optional field.
State: select from the List of Values. Optional field.
Country: select from the List of Values. Optional field.
Zip: type in the zip or postal code for the address. Optional field.
From/To Date: type in the first and last dates of the person's residence at this address. Optional field.
Type: select the type of address from the List of Values. Optional field.
Click Next Block to access the Employment History block.
In the Employment History block, complete the following fields:
Employer: type in the name of the person's last known employer; optional.
Occupation: select the person's occupation from the List of Values. Optional field.
City: select from the List of Values. Optional field.
State: select from the List of Values. Optional field.
Hired / Term[ination] Date: enter the first and last dates for this period of employment using a MM\DD\YYYY format. Optional field.
Hrs/wk: enter the number of hours worked per week. Optional field.;
Salary: enter the amount of money earned. Optional field.
Units: select the List of Values the unit that the salary applies to (daily, monthly, yearly etc.).
Supervisor: type in the name of the person's supervisor. Optional field.
Telephone #: type in the person's telephone number at their place of employment. Optional field.
Click Save.
Click the Add to Offender's Contacts button to close this window and carry the information over to the Personal and Professional Contacts screen.
Click Exit to cancel the information entered in this window.
return to the Personal and Professional Contact screen.
