The Addresses screen is one of many demographic screens used either when booking an offender into an institution, or when performing an intake to activate an offender's record for community work. The Addresses screen records an offender's current resident and mailing addresses.
For institutional work: select Institutions Booking Address from the Main Menu.
OR
For community work: select Community Intake Addresses to access this screen from the Main Menu.
To add a new address, click in a blank row in the Address block.
In the Address block, complete the following fields::
Apartment/Unit #: type in the apartment number and/or street number of the address; optional.
Street: type in the street name; optional.
City/Town: select the name of the town or city from the List of Values; optional. If the name you need is not there, click on the Add City button. This opens the Add to LOV screen, where you can make new entries to the List of Values.
Postal Code: type in the postal or zip code for the address; optional.
State/Province: select the state/province name from the List of Values; optional.
Country: select the name of the country from the List of Values; optional.
Address Type: select the type of address from the List of Values; optional.
Note: You can have the offender's home address copy over to the Personal Contacts screen for any person (contact) that the offender lives with. To do this, you must select type RES(IDENCE) in this field, and fill in the From Date field in the Details block.
Click Next Block to access the Details block.
In the Details block, complete the following fields:
Residence From Date: type in the month and year the offender began residing at the address selected in the Addresses block; optional.
Note: You can have the offender's home address copy over to the Personal Contacts screen for any person (contact) that the offender lives with. To do this, you must select type RES(IDENCE) in the Address block, and fill in this From Date field.
Residence To Date: type in the month and year the offender stopped residing at the address selected in the Addresses block; optional.
Mailing From Date: type in the month and year the offender began using the address selected in the Addresses block as a mailing address; optional.
Mailing To Date: type in the month and year the offender stopped using the address selected in the Addresses block as a mailing address; optional.
Click Next Block to access the Contact Numbers block.
In the Contact Numbers block, complete the following fields:
Type: select from the List of Values the type of telephone number at the selected address; optional.
Number: type in the telephone number, including the area code, for the selected address; optional.
To add more addresses, repeat steps 2 through 7.
Click Save.
Click Next Block to access the next screen in the booking or intake sequence.
