Step 3 for creating a community contact
After you have created an offender-unique ID#, you are ready to perform an intake on the offender.
Performing an intake on an offender creates a contact number and activates the offender file so community service workers can work on the record. The detail you record during the intake sequence becomes the "core" of the offender record. You may use up to seventeen screens (sixteen of them demographic screens) to enter this information:
Intake (activates an offender record for community service staff work)
Identification (physical information - height, weight, etc.)
Address (past and present residential and mailing addresses)
Aliases & Other Identifiers (all known names used by the offender, and identifying documents)
Physical Marks (physical markings or features that help to identify the offender, e.g., scars or tattoos)
Personal/Professional Contacts (any known friends, family and professional contacts)
Victims and Witnesses (information about victims or witnesses associated with the offender)
Family Details (additional information about the offender's family members and marital status)
Alerts (any special measures required in dealing with the offender - medical, security, transport)
Employment History (job history)
Languages (read, written, spoken and understood by the offender)
Health (a general description of the offender's health, with details of particular problems)
Substance Abuse (history)
Finances (current income, expenses, assets and liabilities)
Education (history)
Financial Details (income, expenses, assets and liabilities; community only)
Military History (details of military service, if any)
Intake screens may be linked by a formpath, so you can move between screens without having to use the menu. Beginning with the Intake screen, you can proceed to the next screen in the intake sequence without returning to the main menu.
Remember to save any changes before continuing to the next screen in the sequence.
From the last block on a screen, click the Next Block to access the next screen in the intake sequence.
From the Header block, click the Previous Block to access the last screen in the intake sequence.
The intake sequence on your system may not follow precisely the same order as that outlined here. Your TAG application administrator may set up a different formpath according to user priority, or there may be fewer screens. In either case, the screens will still function as described here.
If the offender has a previous record, some or all of the demographic information may already appear in the screens when you access them after filling in the Intake screen. On data copied over, the I/C indicator bubble-stamp shows the intake event (date/time/user ID of last edit). Move the cursor over the far left column to see it.
