Use this screen to record the details of an incident. You can attach the description to the record of one or more offenders.
To access this screen from the Main Menu, select Institutions Incidents Incident log Incident Details.
In the Incident Details block, fill in the appropriate fields:
Log Number: system generated when the screen is saved.
Occurrence Date/Time: default to the current system date and time. You can change the date to a past date, but not to a future date.
Status: not enterable; defaults to Active when the screen is saved.
Institution: defaults to the caseload institution; select a different location from the List of Values if necessary.
Area: select the institutions's internal location fo the incident from the List of Values; mandatory.
Level: select the specific part of the area where the incident occurred from the List of Values, if applicable; optional.
Occurrence Type: select the type of incident from the List of Values; mandatory.
Description: record a detailed description of the incident; optional.
Reported by: defaults to you, the user ID currently logged on.
Date Reported: defaults to the current date, which you can change; optional.
If repairs are required, click Next Block to the Repair Details block. You can record multiple repairs.
Fill in:
Type: select the type of repair; optional.
Comment: record a comment; optional.
Click Other Details to move to Page 2.
This screen is used as required and is not mandatory. If theofficers and offenders involved in the incident are known, you can record them here.
The Incident block shows the details already recorded. Click Next Block to access the Offenders Involved block.
Complete the following fields in the Offenders Involved block:
[Unique Identifier]#: Click the List of Values arrow to open the Name Search screen.
Query to see the names of all active and inactive offenders across all caseloads.
Select a name or number.
Click Exit to close the Name Search screen and carry the selected number and name over to the Offenders Involved block. You will not be able to select an offender whose institutional admit date is later than the incident date.
Repeat for each additional offender involved in the incident.
Participation: select the offender's role in the incident from the List of Values; mandatory.
Click Next Block to access the Staff Involved block, then complete the following fields:
ID#: select the officer from the List of Values; mandatory. The Name field fills automatically.
Participation: select the officer's role in the incident from the List of Values; mandatory.
Click Save.
You can record a decision against the incident, either at the time you are recording the incident or at a later time. This will close the incident.
If necessary, query to locate the incident record.
Click Next Block to access the Disposition block, then complete the following fields:
Date/Time: defaults to durrent date and time, which you can change; optional.
Type: select the type of disposition from the List of Values; mandatory.
Authorized by: select the authorizing officer from the List of Values; optional. The list shows all the staff members at the institution.
Click Save.
