The Aliases & Other Identifiers screen is one of many demographic screens used either when booking an offender into an institution, or when performing an intake to activate an offender's file for community work.
The Aliases & Other Identifiers screen records all of the offender's known alias names, and designates one name as the working name that TAG displays whenever you open this record. (When you book/intake an offender, the name under which the ID# was assigned automatically becomes the working name.) You can also record additional pieces of identification, correct misspellings in the name or change the birthdate.
For institutional work: if you haven't used a formpath through the
booking sequence, select Institutions

For community work: if you haven't used a formpath through the intake
sequence, select Community

Click on Next Block to move to the Aliases block. TAG automatically displays the offender's current working name in this block.
Fill in these fields for all known aliases:
Last Name: type in the last name of the alias; mandatory.
First Name/Middle Name: type in the first and last names of the alias; optional.
Birthdate: defaults to the date of birth in the Header block; change if required; optional.
Suffix: if the alias has a suffix, such as Jr., or II, type it in; optional.
Name Type: select from the List of Values; optional.
Click on Save.
Working Name checkbox: one name must be selected as the working name. TAG automatically checks the Working Name checkbox next to the same name that is in the Header block. To make another name the working name, select one of the names in the list by clicking on the WN button.
Click on Next Block to move to the Identifiers block.
In the Identifiers block, record the pieces of offender ID that support the name selected in the Aliases block.
Type: select from the List of Values; mandatory.
Number: type in the number on the ID; mandatory.
Issuing Authority: type in the name of the agency issuing the ID; optional.
Click on Save.
From the Identifiers block, click on Next Block to advance to the next screen in the booking or intake sequence.
